Below are the various documents that I had to create so that I can gain ethics approval to get feedback from my target audience on my animations. The first two documents are information sheets that explain what my project is and why I am wanting to gain feedback from children, while the third document is the consent form that the parents or guardians will need to sign to give their child consent to take part in my research.
This first document is the information sheet for the children to read. I have created this to explain my project in more simple terms so that 7-11 years olds will be able to read and understand what it is I am asking of them. I have also included colour and images to this document to make it look more exciting and to help break up all the words.

This second document is the information sheet to the parents/guardians of the children that I wish to take part in my research. This document contains more detail about what my project is and why I am want to collect this research than the child’s information sheet so that the parents have more of an understanding on what I am doing and can elaborate further for their children if required. Because this information sheet is for the parents/guardians it has been laid out in a more formal manner and have all the relevant contact information that the parents/guardians may require.

This final document is the consent form that the parents/guardians will need to sign in order to give their children permission to be a part of my research.

Having completed these documents for the ethics form, my next step while waiting to see if I get approval by the ethics committee will be to continue working on the development of my project and animations so that if I get approved I can begin sending out samples of my work to my target audience so that I can gain feedback on what they think.
